Hormone Therapy After Age 65: What Every Woman Should Know

For decades, hormone therapy (HT) was primarily seen as a short-term solution for managing menopausal symptoms like hot flashes and sleep disturbances. But a new landmark study is reshaping how we view long-term hormone use — especially in women over age 65.

Between 2007 and 2020, researchers analyzed data from more than 10 million senior women enrolled in Medicare. Their goal? To assess the long-term effects of hormone therapy on overall health, chronic disease, and mortality.

The Findings: Reduced Risk in Multiple Areas

The results were both significant and encouraging:

  • Dementia risk decreased by up to 32%
  • Cardiovascular disease risk was significantly reduced
  • Cancers of the breast, colon, and lung were also reduced, each in the teens
  • Osteoporosis-related fractures were significantly lower
  • Overall mortality was lower in women who continued HT past age 65

These are not small findings — they represent meaningful reductions in the most common causes of death and disability among postmenopausal women.

Bioidentical Hormones: A Key Focus

Importantly, the study also included large groups of women using bioidentical hormone therapy, specifically transdermal (topical) estrogen combined with oral micronized progesterone — the same approach many functional and integrative providers (like our clinic) recommend.

What makes this especially impactful is that no increased risk was found for stroke, heart attack, or blood clots when using these lower-risk formulations. In fact, some risks were lower compared to those not using therapy at all.

Is It Too Late to Start?

While earlier initiation of hormone therapy (closer to the onset of menopause) may offer stronger neuroprotective benefits, this study shows that even starting or continuing hormone therapy after age 65 provides measurable protection for the brain, bones, and cardiovascular system.
